PERFORMANCE PLANNING Under the supervision of the clinical coordinator, the LPN assists with direct/indirect patient care according to experience, training, certifications, and demonstrated ability within the limits of the job description. KEY RESPONSIBILITIES / ESSENTIAL FUNCTIONS Collects and documents data on each assigned patient on an ongoing basis to include but not limited to the normal systems review, behavioral, psychological, developmental, and age-related factors. Specific ages: adult. Performs basic and /or advanced nursing tasks as appropriate for the department/unit. Participates in team conferences, care planning and problem identification. Provides and documents patient education based on physical, behavioral, psychological, developmental, and age-related factors, after identifying barriers to learning. Follows safety practices and helps to maintain a safe, clean environment. Records and reports to the physician/clinical coordinator all observed abnormal and/or significant findings to include but not limited to symptoms, abnormal drainage, reactions, responses to treatment, medications, and changes in the patientu2019s condition. Documents all patient care activities and/or observations according to policies and procedures. Attends inservice programs, all mandatory required training/education and unit/department/service meetings. Administer prescribed medications and document date, amount and route. Observe patients, charting and reporting change in patientsu2019 conditions, such as adverse reactions to medication or treatment, and taking any necessary action. Provide basic patient care or treatments, such as taking temperatures or blood pressures, dressing wounds and applying splints. Sterilize equipment and supplies, using germicides. Answer patientsu2019 calls and determine how to assist them. Measure and record patientu2019s vital signs, such as height, weight, temperature, blood pressure, pulse and respiration. Works as part of a healthcare team to assess patient needs, plan and modify care, and implement interventions. Collect samples, such as blood, urine, or sputum from patients, and perform routine laboratory tests on samples. Prepare patients for examinations, tests, or treatments and explain procedures. Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation. Performs compliance requirements as outlined in the Employee Handbook Must adhere to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self. Requires use of electronic mail, time and attendance software, learning management software and intranet. Must adhere to all **MEMBERS ONLY**SIGN UP NOW***. policies and procedures. All other duties as assigned. MINIMUM KNOWLEDGE, SKILLS, EXPERIENCE REQUIRED Holds a current Alabama license and in good standing with the board. u00a0At least one year experience in outpatient/ambulatory setting preferred. Skill mix must include dressing changes/wound care and splint application. Urgent care experience preferred. Must be able to read, write legibly, speak, and comprehend English. WORKING CONDITIONS u00a0 Environmental Includes exposure to human body fluids, disease, infection, lab chemicals and hazard material Requires wearing common protective or safety equipment Physical Physical presence onsite is essential. Hearing and vision must be normal or corrected to within normal range. Able to perform the duties with or without reasonable accommodation. M-Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work. Psychological Contact with others
